HEALTH OFFICE
• The Health Office is located in the Basement of
BASCS. Visitors must first sign in at the Main Office, and then will be
directed to the Health Office.
• Accident and emergency treatment is limited to First Aid by school personnel
at the time of the accident or injury. Further treatment is a parental
responsibility.

• If the child is complaining of illness, the nurse
does an assessment, and the child is allowed to rest in the Health Office if
necessary for as long as 30 minutes or so. If the child has a fever or is
vomiting, or not feeling better after cot rest, a parent (or other emergency
contact) is contacted to take the child home. Children will also be sent home
if they present with symptoms of pinkeye, contagious rash, disruptive coughing,
or head lice.
• If the school nurse is to administer ANY medication, either prescription or
over-the-counter, she must have written permission from both the parent and the
physician. This written permission is only good for the current school year.
The medication must be in the original container and kept in the Health Office.
• Inhalers may be kept on the student's person ONLY with written permission.
• Permission must be on file in the Health Office from the parent and
the physician stating as such.
• Screening procedures include heights and weights (done by physical education
teachers during gym), vision, hearing, and scoliosis (curvature of the spine).

Guidelines for keeping your child
home from school
• fever in the past 24 hours
• vomiting in the past 24 hours
• diarrhea in the past 24 hours
• strep throat until on antibiotics for 24 hours
• bad cold, with a very runny nose, extremely congested, or a bad cough that
would be disruptive, especially if it has kept the child awake at night
• head lice until treated and nit-free
• pink-eye until on antibiotic eye drops for 24 hours
• undiagnosed rash
Reporting an absence: Whenever a student is going to be absent
or late, please call the attendance office at 446-5681 ext. 231. at 8:00 AM You
may call before school hours and leave a voice message if that is more
convenient.
A written absence or tardiness excuse must accompany the child when he/she
returns to school.
An absence will become illegal if a written note is not received in the
attendance office within ten days. Absences from school for vacation or other
family outings are not authorized and must be treated by the school as an
illegal absence.
Please remember to call to inform us about your
child’s absences or tardiness. Please call between the hours of 8 am -10am. You
can also ask to talk to the nurse. Remember to send a written excuse with your
child upon return to school, with name of student, date of absence or
tardiness, and parent signature.
Medication at School......How does
it work?
• Medication will be given by the school nurse or
designated personnel only.
• A physician's prescription is required for all medication given at school;
this includes over the counter medication (Tylenol, cough drops or herbal
remedies)
• Written parental permission should accompany the physician’s prescription.
Physical Forms
Dear Parents/guardians,
New York State law requires that all seventh grade,
tenth grade and special education student’s have a current physical form on
file in the school health office. The physical will be acceptable if done
within the year of school entrance. These forms must be received by October 1st
of the current school year.
All medicine permission forms are to be signed by both the health care
provider and the parent on the approved form. All medicines must be in a
pharmacy container with your child’s name on it. If there are any questions
please contact the health office at 716-446-5681.
Please use the enclosed forms that need to be filled out by your family doctor
and returned to BASCS by Oct 1st.

Varicella or Chicken Pox
As of January 1, 2005, Public Health Law Section
2164 was amended to require: • Students born on or after January 1, 1994, and
who enroll in the 6th grade at the beginning of the 2005-2006 school year must
be immunized against varicella.
